How can I change the name of my 3-year-old son?

The procedure to changing name involves the following 3 steps.

  1. Create an affidavit for change of name. In the case of a minor, the applicant is the guardian (Father/Mother) and the affidavit needs to be provided by the legal guardian.

  2. Publish about the name change in Newspaper

  3. Notify it in State Gazette. For notification in State Gazette, you need to provide the following documents.

    • An undertaking duly signed by the guardian.
    • Original Newspaper
    • Prescribed proforma in duplicate duly typed with the signature of the applicant and two witnesses.
    • CD containing the print matter without witness portion in MS Word and type Name of Guardian in place of a signature.
    • Two self-attested passport size photographs of each applicant and child.
    • A certificate duly signed by the applicant declaring therein that the contents of hardcopy and softcopy are similar.
    • Request letter along with the requisite fee.
