Whom to approach for this service (Designated Officer)?:  Medical Officer of Health/Deputy Health Officer and Superintendents of Major Hospitals

Procedure involved to get this service: 1. Prescribed applications should be submitted with details like date of birth, mother’s name, and hospital where child is born. 2. Receipt is issued for payment 3. Based on the details it will be searched 4. Certificate will be printed and issues to the applicant

Form to be submitted to get this service: Prescribed Birth Certificate application which will be supplied free at the counter.

Who are eligible to get this service: Citizen whose is / child born under BBMP jurisdiction

Documents to be enclosed with the request:

1 .  PRESCRIBED APPLICATION FORM.
2 .  RECEIPT OF PAYMENT MADE.

Fee/Charges to be paid to get the service: For first copy Rs.50, additional copies will be chargeable at Rs.10 each

Maximum number of days to wait to get this service delivered: 7 Working Days

Whom to approach as a appeal (Competent Officer), if the service is not delivered in time or rejected by officer: Joint Director (Statistics)

Maximum number of days to wait to get the decision of the Competent Officer: 15 Working Days

Whom to approach as 2nd appeal (Appellate Authority), if the decision of the Competent officer is not acceptable or not implemented?: Additional Commissioner (Administration)
