The person attaining the age of 18 yrs and above who wishes to get his/her change of name published in the Gazette of India has to submit the following documents.
- An Undertaking duly signed by the applicant
- Original Newspaper with advertisement of change in name
- Prescribed proforma in duplicate duly typed with signature of applicant and two witnesses.
- C.D, containing the print matter without witness portion in MS Word and type old name in place of signature,
- Two self-attested passport size photographs and photocopy of ID proof (self-attested),
- A certificate duly signed by the applicant declaring therein that the contents of hard copy and soft copy are similar.
- Request letter along with the requisite fee.

The following documents are required for updating your name in Passport.

Old Passport in original with self-attested photocopy of its first two and last two pages, including ECR/Non-ECR page (previously ECNR) and the page of observation (if any), made by Passport Issuing Authority and validity extension page, if any, in respect of short validity passport

At least two public/school documents issued in the desired/ applied changed name to ascertain that the applicant has actually changed his/her name

Clippings of two local news papers or the Gazette notification of the concerned State Government, as the case may be
