I was born in the year 1971 at a government hospital in Kolkata. I do not have any birth certificate or hospital documents. How do I apply or get a birth certificate from KMC?

Documents Required for delayed registration of Birth in Kolkata is provided below.

• Written permission from the District Registrar / Additional District Registrar of Births & Deaths in case of a rural area or from the Chief Municipal Health  Officer / Administrator / Chief Executive Officer of a municipal corporation or from the Chairman / Executive Officer / Administrator in case of a municipal area or from the Chairman / Executive Officer of a Cantonment area / Notified area as the case maybe

• Certificate of Hospital or Doctor.

• Affidavit by Petitioner.

• Declaration from 5 local people for granting okay to delayed registration.

• An order from the Executive Magistrate, if the registration of birth happens after one year of its occurrence.

Once you have these documents, you can apply for it either through online or offline means as provided below.

Apply Online

• Login to West Bengal eDistrict portal. After logging to West Bengal eDistrict portal, the Home page appears. It shows the comprehensive list of services under West Bengal eDistrict. The list includes service name and tentative time line to avail the service. The home page contains several panels showing the number of applications for each module of service, such as- Social Welfare Scheme, Registration of Societies, Land Records and Revenue Court Services, Services of Labour Department, Certificates and Licenses.

• Click on ‘Permission for Delayed Registration of Birth’ available under the column ‘District'.

•  ‘Instructions and Requirements of Permission for Delayed Registration of Birth’ page opens up. Click on "Apply".

• Fill up the Applicant's required details such as the details of Child, Father and Mother, Permanent address of Parent, Place of Birth, Address of Parent at the time of Birth, Residence of Mother.

• Provide the details of 5 Local Person granting okay to delayed registration

• After the applicant fills up the application form, click on Save & Next button. Application details will become visible.

• The applicant can either proceed further by attaching supporting documents.

• The applicant can view the application details and supporting document list before the submission of the application form for delayed Registration of Birth.

• Click on "Submit" to submit the application.

• When the application is successfully submitted, the applicant receives an acknowledgment. This contains the application summary and the contact details of the person to be contacted with for further queries.

Apply through Citizen Kiosk/CSC

• Visit nearest Citizen Kiosk/CSC (Tathya-Mitya Kendras) with the required documents.

• Operator will fill the required information on your behalf.
