My COVID PF withdrawal claim got rejected twice stating "name not mentioned in cheque" and incomplete bank details. How to resolve it?

The cheque leaf or passbook page that you enclose with the online form, should reflect the member's name, account number, IFSC of the bank clearly and it should be readable. The same should be available in member's UAN-KYC as well.
