You need to submit a proof of your date of birth (any of Birth Certificate, SSLC, Aadhaar etc) and address proof (any of Ration card, Aadhaar card, bank passbook, Electricity bill, Water Bill etc)to apply for Voter ID card.

If the husband or any other member of the family has previously been enrolled and issued EPIC, he /she may give an undertaking on the photocopy of EPIC about the identity and her residence.
