Following documents are required to register birth in Madhya Pradesh.

Identity Proof of the parents (PAN Card/ Aadhar Card/ Electricity Bill/ Driving License/ Passport)

- After 21 days to within 1 month of birth: Identity proof (Mother and father)

After 1 month to 1 year of birth: District Statistics Officer order + Identity proof (Mother and father)

After 1 year to 15 years of birth : Magistrate’s order + Identity proof (Mother and father)

In case of birth at a private hospital, the hospital provides a Hospital Discharge card. The applicant submits the hospital discharge card and other supporting documents to the ward office while collecting the Birth Registration Certificate.

In case of a birth at Govt. hospital, the birth registration can be done directly at the Govt. Hospital and the applicant need not visit the ULB ward office. Only Govt. Hospitals have the authority to register and provide Birth Registration Certificate to the applicant whose child is born in that Govt. Hospital.
How to Apply Online for a Birth Certificate in Madhya Pradesh?
To apply online for a birth certificate in case of domiciliary event, follow these steps

Visit the Civil Registration System website and fill the mandatory information to register yourself for reporting of an event.

At the time of sign-up, users will be able to register for the functional registration units only i.e. the registration units where this centralized application is being used for registration of births and and in the drop down list of registration unit at sign-up page, only these RUs will get listed because it has not been implemented in all the RUs of the country.

After successful login, users need to fill the complete birth reporting form (both legal as well as statistical portion) to the best of their knowledge and belief.

Get the print out of the application and forward to the concerned Registrar BY HAND at the given address (address will appear on the bottom of the print out of application) along with the requisite documents.

After successful submission of the application, users will get a confirmation mail on the provided email id.

The users will be informed about the status of the application through e-mail immediately after receipt of the application by the concerned registrar.

The details of the registered event will be informed through e-mail or it can be checked anytime from the home page of the portal by quoting the Application Reference Number.
Prescribed Time Period for Registration of Birth in Madhya Pradesh
The normal period of 21 days (from the date of occurrence) has been prescribed for reporting the birth, death and stillbirth events.
