How to get Death Certificate in Nagaland?

Death certificate of Nagaland is an official document issued by a doctor, which declares the cause of death, location of death, time of death and some other personal information about the deceased.

Eligibility Criteria

The following are the eligibility criteria for Death Certificate in Nagaland.

  • Death certificate will be issued to any citizen or any foreigner's legal heir, the death of whose relative takes place in Nagaland.

  • Parents and relatives of the deceased are eligible.

Documents Required

The following are the documents required for Death Certificate in Nagaland.

  • Application form

  • Proof of birth of the deceased – Birth certificate/SSLC certificate

  • Copy of the ration card

  • Medical Certification of Causes of Death, if required

  • Aadhaar card of deceased (if available) and applicant

  • ID card of the applicant

  • Residential Proof

  • Date and time of death

  • An affidavit specifying the date and time of death or death slip by doctor

  • Copy of Receipt from crematorium or burial ground

  • If there is no record for death from the hospital, then certificate obtained from village head / Regional Councillor / MLA / MP / MBS with stamp and signature along with a signature of any one of the doctor should be submitted

  • If you are an existing user login using User ID and password. Otherwise, click on “Sign Up”.

  • After SignIn, the application form for Death Certificate in Nagaland will appear

  • Click on “E-District Services”.

  • Select “Death Certificate”.

  • After filling up the form, USERs will be redirected to the Payment Gateway for paying the necessary Fess.

  • Non-payment of Service Fees will be considered an incomplete application and the Application will not be submitted for further processing by the Service Providing Department.

  • The district Payment Gateway has Transaction Charges and includes applicable GST which will be charged to the Applicant. This Transaction Fee is fixed for all Modes of Payment i.e. Debit/Credit Cards, NetBanking, etc.
