How to do Late Registration of Birth and Death in Andhra Pradesh?

Late Registration of Birth/Death can be applied online from the Meeseva portal. For this, you need to register in the Meeseva portal. Follow the below steps to register in the Meeseva portal.

Meeseva Online Registration

  • Fill the required details such as Desired Login ID, Password, Secret Question etc.

  • Enter contact information such as Email, Confirm Email, Alternate Email and Mobile no.

Meeseva Contact Details

  • Enter personal information details such as Aadhaar No, First Name, Last Name, Gender, Date of Birth, Address Line1, Address Line2, Country, State, City and Pin Code.

  • Click on Submit button for registration submission.

  • After submission, a One Time Password will be sent to your mobile number.

  • Enter OTP and click on Confirm button to confirm the registration.

  • Click on Revenue Department.

  • Select "Late Registration of Birth/Death" under the List of services.

  • Upload the scanned copies of the documents mentioned in the prescribed format.

  • Make the required payment by clicking ‘Show Payment’. 

  • On confirmation, the acknowledgement receipt will be generated along with the Certificate Delivery Date.

  • The application will be processed, and the status will be intimated to the applicant.

  • Once the application is approved, the citizen will receive a Message like “Your Request for D-Form Patta has been Accepted Vide Application No: ICXXXXXX Transaction No TAICXXXXXX Please collects your Certificate at MeeSeva-XXXXXXXX”.

  • Late Registration of Birth/Death Certificate will be dispatched either through courier to the citizen’s address, if the delivery type is Speed Post or if the delivery type is manual, the citizen can collect it from franchisee where he/she applied for the certificate.
