Following documents are required to apply for OBC caste certificate in Madhya Pradesh for OBC Category.

For caste confirmation- Provide photocopies of records of immovable property (land/plot/house registry or any other revenue record, etc.) in the names of family members (grandfather/grandmother/great-grandfather/great-grandmother/father/mother/uncle/brother) in which the caste is mentioned. Alternatively, provide a copy of the caste certificate issued by the Sub Divisional Officer (Revenue) for any family member (father/uncle/brother/sister/grandfather/other blood relative on the paternal side) issued after 1996.

To confirm residence status in Madhya Pradesh in 1984 or earlier, or subsequently, provide education/service/voter introduction certificate or photocopies of records of immovable property (land/plot/house registry or any other revenue record, etc.) in the names of family members (grandfather/grandmother/great-grandfather/great-grandmother/father/mother/uncle/brother) in which the caste is mentioned.

Photocopies of self-attested educational qualification certificates of the applicant.

Attach a declaration letter regarding the relationship between caste and residence dates.

For income verification, attach a certificate issued by the Deputy Tehsildar/Appointing Authority or any affidavit or oath letter.
- Include the area and location of agricultural land (owned by mother/father/adult children) in the family's possession, certified by a revenue officer (not below the rank of Tehsildar).
