UDID Card

UDID Card or Unique Disability Identity card is an initiative by the Government of India with a view of creating a National Database for PwDs, and to issue a Unique Disability Identity Card to each person with disabilities.

Types of UDID Card

Three types of coloured based UDID Cards are issued based on the severity of the disability-

  1. White Card: When the disability percentage of a PwD is below 40%.

  2. Yellow Card: When the disability percentage of a PwD is above 40% but on or below 80%.

  3. Blue Card: When the disability percentage of a PwD is above 80%.

Eligibility Criteria for UDID card

As per the Persons with Disabilities Act,1995 only those with disabilities can apply for a UDID Card.

UDID recognises 21 types of disabilities as mentioned in RPwD Act 2016.

Here are a list of few of the Disabilities:

  • Blindness: “Blindness” refers to a condition where a person suffers from any of the following conditions namely:-

    • Total absence of sight; or

    • Visual acuity not exceeding 6/60 or 20/200 (Snellen) in the better eye with correcting lenses; or

    • Limitation of the field of vision subtending an angle of 20 degree or worse;

  • Cerebral Palsy: “Cerebral Palsy” means a group of non-progressive conditions of a person characterized by abnormal motor control posture resulting from brain insult or injuries occurring in the pre-natal, peri-natal or infant period of development;

  • Low vision: ”Low Vision” means a person with impairment of visual functioning even after treatment of standard refractive correction but who uses or is potentially capable of using vision for the planning or execution of  a task with appropriate assistive device;

  • Locomotor Disability: “Locomotor disability” means disability of the bones, joints or muscles leading to substantial restriction of the movement of the limbs or nay form of cerebral palsy;

  • Leprosy-cured: “Leprosy-cured person” means person who has been cured of leprosy but is suffering from-

    • Loss of sensation in hands or feet as well as loss of sensation and paresis in the eye and eye-lid but with no manifest deformity;

    • Manifest deformity and paresis but having sufficient mobility in their hands and feet to enable them to engage in normal economic activity;

    • Extreme physical deformity as well as advanced age which prevents him from undertaking ang gainful occupation, and the expression ‘Leprosy Cured” shall be construed accordingly;

  • Mental Retardation: “Mental retardation” means a conditions of arrested or incomplete development of mind of a person which is specially characterized by sub normality of intelligence;

  • Mental Illness: “Mental illness” means any mental disorder other than Mental retardation

  • Hearing Impairment: “Hearing Impairment” means loss of sixty decibels or more in the better ear in the conversational range of frequencies

Documents required to apply for a UDID card

The following documents are required to apply for a UDID card

  • Scanned copy of recent color photo. 

  • Scanned image of signature (Optional).

  • Scanned copy of Address Proof (Aadhar/Driving License/State Domicile etc).

  • Scanned copy of Identity Proof (Aadhar Card/PAN Card/Driving License etc).

  • Scanned copy of Disability Certificate (Only for those Persons with Disability who have been issued disability Certificate is by the competent Authority).

How to register on the UDID website?

Follow the below steps to register on the UDID website

  • Visit the UDID Web Portal.

  • Click on the ‘Register’ option.

  • Here, four types of details are required:
    • Personal Details including Address

    • Disability Details

    • Employment Details

    • Identity Details
  • Carefully fill all the required details and submit. 

How to apply online for UDID card?

Follow the below steps to apply online for UDID card.

  • Visit the UDID Web Portal.

  • Using credentials, log in to the system using your enrolment number and Date of Birth

  • Click on “Apply online for Disability Certificate”. Carefully read the instructions and fill up the online application.

  • Upload color passport photo and other requisite documents like Income Proof, Identity Proof and SC/ST/OBC proof as required.

  • The data is submitted to the CMO Office/Medical Authority.

  • CMO Office/Medical Authority verifies data.

  • CMO Office/Medical Authority assigns the concerned specialist(s) for assessment.

  • Specialist Doctor assesses disability of Person with Disability and gives opinion on disability.

  • The Medical Board reviews the case and assigns a disability percentage. CMO Office prepares Disability Certificate and generates UDID and Disability Certificate.

  • UDID datasheet goes for UDID Card printing and Card dispatched to PwD. 

How to track the status of UDID Card?

Follow the below steps to track the status of UDID card.

  • Visit the UDID Web Portal.

  • Select the option “Track Your Application Status”.

  • Enter your Enrolment / UDID / Request Number / Mobile Number / Aadhaar Number.

How to download your UDID Card?

Follow the below steps to download UDID card.

  • Visit the UDID Web Portal.

  • Click on the ‘Download your e- Disability or e-UDID card’ option.

  • Login using your enrolment number or UDID number.

  • Download your UDID Card.

What is the validity of a UDID Card?

The UDID card has two types of validity- Permanent UDID card and Temporary UDID Card.

  1. Permanent UDID: The competent medical authority issues Permanent UDID Card considering there is no progress or regress in the disability. 

  2. Temporary UDID : The competent medical authority issues Temporary UDID Card considering there is a progress or a regress in the disability.
