• Affidavit for vehicle usage during Corona Crisis in Kerala (PDF)
• Operation Permit for Staff (For essential goods & services) - Operation permit for staff working in shops and establishments dealing with essential goods and services and to provide essential services to the dependent employees of any organization can be availed here.
• Vehicle Permit for Essential Services - You can apply for Vehicle Permit for transportation of Essential Goods and to facilitate essential services declared by the Government. This service can be availed by owners of shops/establishments by declaring their license of their establishment. This can also be used by people who want to avail of Emergency Services. An SMS will be received in the registered mobile number with a link to download the permit with QR code.
• Self Declaration - Self declaration can be used by the public for emergency travel and travel to fetch essential goods and avail essential services. Submit the details of your travel requirement by registering your mobile number. An SMS will be received in the registered mobile number with a link to download the self-declaration with QR code.
• Complaints - Register your complaint/request related to Covid19 prevention in the district. Public can register complaints/request on difficulties faced in accessing health services, PDS, food. You can also register complaints on issues related to people violating quarantine and lock down norms. Anyone facing any issue anywhere can file the request here.
